Consent to Telehealth

By signing, submitting, or otherwise opting to agree to this consent, you understand and agree with the following terms.

Telehealth and telemedicine involve the use of electronic communications to enable authorized healthcare providers at different locations to share individual patient medical information for the purpose of improving patient care. You consent to receive treatment via telehealth from authorized providers associated with the Kind MD platform.

Authorized providers may include primary care practitioners, specialists or subspecialists, nurse practitioners, registered nurses, medical assistants, and other healthcare providers who are part of your clinical care team. In addition to yourself and members of your clinical care team, you may choose to have family members, caregivers, legal representatives, or guardians join and participate in the telehealth service, and you may agree to share your personal information with them if you choose to have them participate.

The information exchanged through telehealth may be used for diagnosis, therapy, follow-up, education, treatment planning, or other care-related purposes.

Privacy and Use of Health Information

The laws that protect the privacy and confidentiality of health and care information also apply to telehealth. Information obtained during telehealth services that identifies you will only be provided to someone with your consent, except as permitted or required for treatment, payment, healthcare operations, or by law.

By agreeing to use telehealth services, you consent to the sharing of your protected health information with certain third parties as more fully described in Kind MD's Privacy Policy. You understand and expressly consent to Kind MD, its service providers, and authorized care partners obtaining, using, storing, and disseminating information about you as necessary to provide telehealth services and as specified in the Privacy Policy.

Communications and Message Review

You understand that you must check the website, portal, email, text messages, or other communication channels made available to you because this is how authorized providers and the care team may communicate important information to you. If you do not check these channels regularly, your care may be delayed.

If you have non-urgent questions relating to your care, you may message the authorized provider or care team through the available website or portal. You understand that the provider or care team may not review and respond to messages until the next business day depending on when the message was sent.

Technology Risks and Limitations

Telehealth sessions may not always be possible. Disruptions of signals or problems with internet or telecommunications infrastructure may cause broadcast and reception problems, including poor picture or sound quality, dropped connections, or audio interference, that prevent effective interaction between the authorized provider, participant, patient, or care team.

You understand and agree that the health information you provide at the time of your telehealth service may be the only source of health information used by the authorized provider during your evaluation and treatment at that time, and that the authorized provider may not have access to your full medical record or information.

Risks, Benefits, and Alternatives

You understand that you will be given information about tests, treatments, and procedures, as applicable, including the benefits, risks, possible problems or complications, and alternate choices for your medical care through the telehealth visit.

Telehealth is not a substitute for emergency care. If you are experiencing an emergency or urgent medical condition, call 911 or seek immediate in-person medical attention.

Right to Withhold or Withdraw Consent

You have the right to withhold or withdraw consent to the use of telehealth services at any time and seek traditional in-person clinical services. You understand that withdrawing consent for telehealth will not affect any future services or care benefits to which you are otherwise entitled.

Consent: By continuing to use Kind MD telehealth services, submitting an intake form, participating in provider communications, or accepting this consent electronically, you consent to the use of telehealth in the provision of care and agree to the terms above.
